- This position is 40 hours a week 3-1130p every other Saturday
730-4 plus weekday and weekend call.
- Job Requirements
- Primary Source Verification of professional license and/or
certification, as required for the position ran within 30 days of
start updated upon expiration. Attestations not allowed. Needs to
be rerun every 2 years with RN license renewal & upon expiration.
Temporary Licenses are NOT accepted - must have a valid permanent
license.
- Proof of vaccine series or completion and approval of client
medical or religious exemption form. Preferred, not required.
- Positive MMR titers ONLY from within the last 15 years. If low
titer was completed within 30 days prior to start, only 1 booster
is required to start and 2nd booster must be given 30 days after
1st dose. This is the only scenario that a booster(s) will be
approved. After the booster series is completed, a new titer must
be drawn 6 weeks after the 2nd booster to show a new positive
titer. Please add into "Certified Until Date" in the technology to
reflect 30 days after 1st booster administered. Please add into
"Certified Until Date" in the technology to reflect 6 weeks after
2nd booster administered.
- Completed within 1 year of start updated annually. Cannot be

- Will accept one of the following: (a) Proof of full Hepatitis B
series (three (3) vaccines) completed (b) Must have at least two
(2) vaccines from the series completed, or (c) signed declination,
agency form acceptable. No childhood vaccines allowed. Positive
titers also accepted within 15 years.
- If positive, CXR within 5 years and current Questionnaire
- Positive Varicella titers ONLY from within the last 15 years.

must be provided from an accredited organization.

- Clinical Only: Skill specific required to be complete within 1

- Competency Exams for the relevant department. Must score at

- 80% passing score required, Completed within 1 year prior to
start. HIPAA, Joint Commission-NPSG, OSHA Topics. CORE 1, 2 and 3
must be uploaded. Attestations not allowed. Only needs to be done
once upon initial hire. This is not an annual requirement.
- Physical Exam completed within one year of initial start date.
Needs to be a Pre-Employment exam statement signed and must state
that traveler is able to work without restrictions. Attestations
not allowed. This is not an annual requirement.
- Proof of vaccine from Oct-March or exemptions must be submitted
for approval. No declinations allowed. If exemption is needed for
medical or religious reasons only, signed UH Exemption Form and UH
Masking Attestation Form required to be signed. For medical
reasons, a statement from a physician is required along with signed
UH forms. Document can be found in document library.
- 2-Step TB test with negative reads within the last 6 months.
Either true PPD 2-step or QuantiFERON/TSPOT is acceptable. This is
an annual requirement. *If positive, clear chest x-ray and current
TB questionnaire required. Chest X-Rays valid for 5 years.
- Resume to include Education and Work History, 2 Professional
References required within the last 2 years, and Job Application to
be completed by the candidate. Please use agency form.
- Shoulders up, plain background, headshot. Upload as JPEG
only.
- Ran within 6 months of Start. Search includes: SSN Trace and
County searches for the last 7 years of locations lived/worked/in
school. National Criminal Sweep to also include NSO. Only need to
complete a new BGC if break in assignment 6m+. *Background Hits -
Traveler must provide a written summary of the incident - upload in
Einstein II for review with background report all in one PDF.

allowed. Must be on lab report. If positive, include MRO report. If
there is a break in assignment greater than 60 days, then a new 10
panel UDS must be performed. This is not an annual
requirement.
- Safety and Compliance Training modules emailed to agency POC by
UHS after acceptance. Candidates need to complete module prior to
clearance. Once complete, please upload the certificate of
completion.
- For Ohio residents to meet fingerprint requirements, please
upload FCRA-Compliant National Database Criminal background check.
